Position Summary

We are seeking a compassionate and organized Targeted Case Manager (TCM) to join our IDD team. The TCM will coordinate, advocate, and monitor services for individuals with IDD, ensuring their needs and goals are met in accordance with Kansas HCBS waiver requirements and CDDO guidelines.

Key Responsibilities

• Develop, implement, and monitor Person-Centered Support Plans (PCSPs) in compliance with Kansas regulations.
• Assist individuals and families with accessing community resources, benefits, and services.
• Advocate for individuals’ rights and ensure informed choice in service planning.
• Maintain accurate and timely documentation in accordance with KDADS, MCO, and CDDO standards.
• Coordinate with providers, schools, medical professionals, and other agencies to ensure holistic supports.
• Conduct required visits, including face-to-face meetings, per state and MCO guidelines.
• Report and respond to concerns regarding health, safety, abuse, neglect, or exploitation.

Qualifications

• Required: Bachelor’s degree in human services field (Social Work, Psychology, Special Education, or related).
• Preferred: Experience working with individuals with IDD or in case management.
• Must meet KDADS Targeted Case Manager qualifications.
• Strong communication, organizational, and advocacy skills.
• Valid driver’s license, reliable transportation, and ability to travel within service areas.
• Ability to pass all required background checks and screenings.

Position Summary

We are hiring caring and dependable Personal Care Attendants (PCAs) to provide direct, hands-on support to individuals in their homes. PCAs help clients with daily living tasks, promote independence, and ensure health and safety.

Key Responsibilities

• Provide assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
• Assist with instrumental activities of daily living (IADLs) including meal preparation, light housekeeping, laundry, and errands.
• Support individuals in community access, social activities, and skill development.
• Follow the Person-Centered Service Plan and document daily tasks and progress.
• Report changes in condition, safety concerns, or incidents to supervisors immediately.
• Ensure client dignity, rights, and confidentiality at all times.

Qualifications

• Must be at least 18 years old.
• Ability to pass Kansas required background checks 
• Reliable transportation and willingness to travel to clients’ homes.
• Physical ability to lift, transfer, and assist clients as needed.

Position Summary

Aspire Home Health is seeking a compassionate, dependable, and organized Licensed Practical Nurse (LPN) or Registered Nurse (RN) to join our home health team.

The Home Health Nurse will provide nursing oversight and support for clients receiving services in their homes. This position includes completing client visits, monitoring changes in health and condition, supervising direct-care staff, completing required documentation, and communicating with clients, families, physicians, case managers, and the Aspire management team as appropriate.

This is an excellent opportunity for a nurse who enjoys working independently, building relationships with clients, and providing care within the community.

Key Responsibilities

• Complete client Start of Care (SOC), wellness, and supervisory visits as assigned.
• Complete nursing assessments and required clinical documentation accurately and timely.
• Monitor clients for changes in health, condition, safety, or service needs.
• Immediately report and appropriately respond to significant changes in condition, hospitalizations, incidents, or other health and safety concerns.
• Provide supervision and oversight of direct-care employees providing services in clients’ homes.
• Review client care needs and ensure services are consistent with established plans of care.
• Communicate regularly with clients, families, direct-care staff, case managers, physicians, and Aspire management as appropriate.
• Participate in nursing and interdisciplinary team meetings.
• Assist with client onboarding and coordination of services.
• Provide client and caregiver education as appropriate.
• Maintain client confidentiality and accurate nursing records.
• Follow Aspire Home Health policies and procedures and applicable state, payer, and program requirements.
• Assist with additional nursing and quality-control responsibilities as assigned.

Qualifications

• Current and unrestricted Kansas LPN or RN license required.
• Graduate of an approved nursing program.
• Previous home health, home care, long-term care, case management, community nursing, or similar experience preferred.
• Strong nursing assessment, documentation, and communication skills.
• Ability to work independently and appropriately prioritize client needs.
• Comfortable traveling to clients’ homes throughout the assigned service area.
• Valid driver’s license, reliable transportation, and current automobile insurance.
• Ability to use electronic documentation and communication systems.
• Must successfully complete all required background checks and employment screenings.
• Must maintain all professional credentials and certifications required for the position.
