Frail and Elderly

Frail and Elderly (HCBS-FE) serves the elderly over 65 years of age as an alternative to a nursing home including help with personal care and household tasks.

Physical Disability

Physical Disability (HCBS-PD) serves individuals 16 to 65 years of age whose disability requires them to need help with performing daily living activities.

Traumatic Brain Injury

Traumatic Brain Injury (HCBS-BI) serves individuals age birth-64 years old with a documented medical diagnosis of an acquired or traumatic brain injury to remain independent with their living
