Targeted Case Manager – Intellectual & Developmental Disabilities (IDD)

Position Summary
We are seeking a compassionate and organized Targeted Case Manager (TCM) to join our IDD team. The TCM will coordinate, advocate, and monitor services for individuals with IDD, ensuring their needs and goals are met in accordance with Kansas HCBS waiver requirements and CDDO guidelines.
Key Responsibilities
- Develop, implement, and monitor Person-Centered Support Plans (PCSPs) in compliance with Kansas regulations.
- Assist individuals and families with accessing community resources, benefits, and services.
- Advocate for individuals’ rights and ensure informed choice in service planning.
- Maintain accurate and timely documentation in accordance with KDADS, MCO, and CDDO standards.
- Coordinate with providers, schools, medical professionals, and other agencies to ensure holistic supports.
- Conduct required visits, including face-to-face meetings, per state and MCO guidelines.
- Report and respond to concerns regarding health, safety, abuse, neglect, or exploitation.
Qualifications
- Required: Bachelor’s degree in human services field (Social Work, Psychology, Special Education, or related).
- Preferred:Â Experience working with individuals with IDD or in case management.
- Must meet KDADS Targeted Case Manager qualifications.
- Strong communication, organizational, and advocacy skills.
- Valid driver’s license, reliable transportation, and ability to travel within service areas.
- Ability to pass all required background checks and screenings.

Position Summary
We are hiring caring and dependable Personal Care Attendants (PCAs) to provide direct, hands-on support to individuals in their homes. PCAs help clients with daily living tasks, promote independence, and ensure health and safety.
Key Responsibilities
- Provide assistance with activities of daily living (ADLs) such as bathing, dressing, grooming, toileting, and mobility.
- Assist with instrumental activities of daily living (IADLs) including meal preparation, light housekeeping, laundry, and errands.
- Support individuals in community access, social activities, and skill development.
- Follow the Person-Centered Service Plan and document daily tasks and progress.
- Report changes in condition, safety concerns, or incidents to supervisors immediately.
- Ensure client dignity, rights, and confidentiality at all times.
Qualifications
- Must be at least 18 years old.
- Ability to pass Kansas required background checksÂ
- Reliable transportation and willingness to travel to clients’ homes.
- Physical ability to lift, transfer, and assist clients as needed.
- Compassion, patience, and a strong commitment to supporting others.
- Prior caregiving or healthcare experience.
